Fees Structure & Office Policies

Equine:

Exam only – $35.00. Includes visual inspection, gait analysis, manual and motion palpating of the spine and joints. No treatment.

New Horse – $150.00. Includes complete history, exam, and chiropractic only.

Follow up visits (Chiropractic only) – $115.00

10 Visits chiropractic package- $950.00 (can be applied to multiple horses).

Chiropractic + Muscle Balancing- $150.00 Includes chiropractic and muscle, tendon, and fascia balancing/release.

Chiropractic + Muscle Balancing + Emotional/Trauma Release – $175.00

Emotional/Trauma (mild to moderate cases) Release (1hr.) – $300.00

Horse and Owner(Rider) Balancing – Physical, Emotional, and. $225.00
Energetic balancing between you and your horse

* Equine services are mobile (I travel to you).
* Dr. Corey reserves the right to refuse working on extreme behavioral or traumatized horses for the safety of the horse and everyone else around.
* Trauma and Emotional release are for mild to moderate cases. Not for extreme cases.

Canine:

Scheduling:

Equine

Dr. Corey sees horses on Mondays and Fridays and every other Saturday. To schedule, please call Dr. Corey’s cell phone (801)-866-7501 to set your appointment time. Please specify the number of horses that you want treated so he can allow for optimal time for each animal as well as travel time. If you have a dog/s that needs to be treated, Dr. Corey can treat them along with your horses. (No need to bring your dog into the office when he’s traveling to treat your horses.)
Although we do book appointments at set times, please allow us a 30 minute grace for elements that are beyond our control. We will contact you directly if we are running behind or ahead of schedule.

Canine

To schedule, call Dr. Corey’s office, in Ogden, at (801)-476-1752. Dr. Corey treats humans and small animals Tuesday, Wednesday, and Thursday.

Terms & Conditions

Denial of Services

We reserve the right to deny services should there be any concern for the animal’s wellbeing.

Cancellations

We require a 24 hour notice for cancellations. Missed appointments without a 24 hour notice will result in a $100.00 missed appointment fee for each horse and $50.00 for each small animal. We understand that weather can be an issue for both parties when Dr. Corey does mobile visits. If weather is an issue, no cancellation fee will be assessed.

Payment: Payment is due at the time of service. We accept cash, credit cards, check Venmo, Square, and PayPal. Dr. Corey does not accept any insurance plans. Should you not be able to be present for the service, we request you please allow payment information to be saved on file BEFORE the time of your visit.

Termination: Ending relationships can be difficult. Dr. Corey may terminate your treatment after appropriate discussion with you and a termination process if it’s determined that professional boundaries are not being upheld, the treatment is not being effectively used, or if you are in default on payment. We will not terminate the therapeutic relationship without first discussing and exploring the reasons and purpose for termination. If treatment is terminated for any reason or you request another provider, we will provide you with a list of other qualified professionals. You may also choose someone on your own or from another referral source. Should you fail to schedule another appointment with me (within in 1 years time), we will consider the professional relationship discontinued.
